We are dedicated to providing the most comfortable, effective, and

Please read the following important information to get acquainted with our office:



General Information and Office Policies

Dental Emergencies: Established patients of All Ages Dental Spa experiencing a dental emergency should call our office. For all other emergencies, please call 911.
If you have a regular appointment and you are experiencing delays in being seen, we apologize in advance. We may be attending to an emergency patient. Please, be assured you will be seen by the doctor as promptly as possible, and you will receive the same careful attention as those who came before you.
Payment: Full payment is due at time of service. Our Financial Policy will be reviewed during your first appointment. Pre-treatment estimates can be submitted to your insurance company upon request.
For your convenience, we accept: Visa, MasterCard, checks, cash, and debit cards.

(Bank fees are automatically added onto patients balance for their returned checks.)



Insurance: For your convenience and as a courtesy, we submit your dental claims if your plan lists Dr. Totzke as a provider. You are responsible for paying fees denied by your Insurance as well as for understanding your dental benefits under your dental insurance policy.
Follow-Up Visits & Insurance: You may change insurance, change plans, or your insurance coverage may change between the time you first contact us and your future appointments. We have no control or knowledge of these events, as the insurance underwriters do not provide us with updates.
Uncertain whether there have been changes made in your policy coverage?

Please call your insurance underwriter before your consultation, and

notify us promptly.

You are ultimately responsible for fees not collected from your insurance company.
Appointments: Visits are by appointment only. Our Patient Service Coordinator will offer you the first adequate time-slot available to give you our full attention. Cancellations made with less than 24 business hours notice to scheduled appointment time automatically trigger a $39 rescheduling fee.
Office Hours: Mon - Wed - Thursday: 8am to 5pm (closed: 1pm to 2pm).

Tue: 8am to 8pm

Friday: 8am to 2pm
Cell Phones, Toys: In an effort to provide a comfortable, courteous and efficient environment for all, including visiting children, we ask all visitors to turn all cell phones, pagers and “noise makers” off.
