Dear Parent(s)Guardian(s),
The attached Health and Dental forms are to be completed by you and your child’s physician/dentist/eye doctor. When completed, please return them to the school office with your registration papers.. We recommend that you call your Doctor/Dentist NOW to make your appointments so as to avoid the summer rush. Please be sure to complete and sign the top portion and Health History of the second page of the Health examination Form.

The following is an explanation of what is required for FALL OF 2014-2015 SCHOOL YEAR:
Pre-School: All new pre-school students are required to have a physical examination with updated immunizations completed within 1 year of entering Pre-school. Students 4 years of age or older need DPT/OPV booster or a note from their Doctor stating when immunizations will be given. Completed PREVNAR Series (NEW) SECOND YEAR PRE-SCHOOL STUDENTS DO NOT NEED REPEAT PHYSICAL EXAMS.
Kindergarten: All kindergarten students are required to have a physical examination completed within 1 year prior to entering Kindergarten (with updated immunizations). Dental examinations are required for kindergarten students. Eye exams are also required. Physical, dental and eye examinations must be completed prior to the first day of school.
Grade 2: Dental examinations are required for all 2nd grade students.

Grades 1-5: New students must have their Health records transferred to St. Paul’s. This must include physical exam, (immunization record) and dental exam completed in kindergarten and second grade. If these are not available, physical examination with immunizations updated and dental examination must be completed prior to date of entrance.
Grade 6: All 6th grade students are required to have a physical examination with updated immunizations completed within one year prior to the first day of 6th grade. Dental examinations are also required for all 6th grade students. Physical and Dental examinations must be completed prior to the first day of school. Whooping Cough as of 2012 fall. Proof one dose of T DAP VACCINE.
Grade 7 - 8: New students must have their Health Records transferred to St. Paul’s. This must include physical examination, immunization records and dental examination from 6th grade or a later date. If these are not available, physical examination, with immunizations updated and dental examination must be completed prior to date of entrance. Whooping Cough is required 6-12th grades. Proof one dose of T DAP VACCINE is required.

New: K-8: Needs 2 doses MMR

PS -12: Needs 1 dose VARICELLA VACINE of which (k,6,9) 2 DOSES

PS,PK, 6-12: Needs HEPETITIS B
Sports Physicals: Mandatory all student athletes have a completed sports physical form and proof of insurance on file in the school office prior to participating in any tryouts and/or school sponsored athletics. This is true if you plan to participate in interscholastic or intramural sports.

Please note: this requirement is already part of the 6th grade physical. 6th is the only grade level that does not need a separate sports form.
ALL PHYSICAL FORMS MUST BE TURNED IN TO THE SCHOOL OFFICE BY AUGUST 1, 2014
