Mountain Shades charges a small fee for shipping and handling. There is $5 charge for children’s items, $10 for items with a MSRP of $25 or less; $15 charge for items with a MSRP of over $25. We will charge your card upon receiving this form along with the sunglasses you are claiming warranty on. If payment information is not provided, no replacement request can be processed.

If you are unable to ship your Mountain Shades or we did not receive them, we will not be able to honor your warranty. Please note that Mountain Shades is not responsible for lost or misdirected packages. We strongly encourage you to send your product through a traceable method such as FedEx or UPS.
