PLEASE NOTE: It is a legal requirement that you purchase a USA Wrestling Membership card before practicing in our club. This card is also required for you to wrestle at any tournaments. This card can be purchased online at www.usawmembership.com. The cost of a Full Membership is $45.

If my child needs medical treatment while wrestling, it is my wish that treatment be started while efforts are being made to contact persons listed on my registration forms. I consent to any medical procedures that a physician deems needed or medically necessary. I accept responsibility for all costs related to my child's care and transportation. Not holding Choker Wrestling Club, Sports Coaching Associates or its representatives responsible for costs.

What should I wear to practice? All wrestlers should come to practice in either practice singlets, t-shirts, shorts, sweats, or athletic pants, and wrestling shoes. And although not required at practice you should bring your headgear with you.
Bring a bottle of water and a good attitude to practice.
Nails: Nails must be clipped and kept short for wrestling practice and tournaments. This will be checked at skin check at tournaments.
RINGWORM: Wrestlers can minimize the chance of contracting or spreading ringworm to other wrestlers if they take a shower after each practice and after each tournament using an antibacterial soap. If ringworm is present, also use an antifungal cream such as: Lotrimin or Lamisil which can be found at your local Walmart, Target, Rite Aid etc. WRESTLERS DO NOT GO OUT ON THE MAT WITH RINGWORM, OR ANY TYPE OF STAPH OR SKIN INFECTION.
Wrestlers are not allowed back on the mats until a Doctor or a Choker Wrestling Club coach clears them. This is for everyone’s protection. At tournaments, there will be a skin check done at weigh-ins, and if ringworm, staph, or other type of skin infection is found, the wrestler will not be allowed to participate.

As lawful consideration for my minor child being permitted to participate in the Choker Wrestling Club program, camps, clinics, or any other activity at the Choker Wrestling Center. I agree that neither my minor child nor I will make a claim against, sue, attach the property of or prosecute Choker Wrestling Club, Sports Coaching Associates and their agents, sponsors, building contractors, suppliers, and volunteers for damages for death, personal injury or property damage which my minor child may sustain as a result of my child's participation in these sporting activities. This release is intended to discharge in advance Choker Wrestling Club, Sports Coaching Associates their agents, sponsors, building contractors, suppliers, and volunteers from and against any and all liability, including for negligent action arising out of or connected in any way with my minor child's participation in the sports league, program, camps, clinic or any other activity except for liability that may arise out of the willful or wanton misconduct of Choker Wrestling Club and their agents, sponsors and volunteers.
I further understand that sports involve physical contact between players, that serious accidents occasionally occur during such sporting activities, and that participants in such sporting activities occasionally sustain serious personal injuries (including death) and/or property damage, as a consequence thereof. Knowing the risks of participation, nevertheless, I hereby agree that my minor child and I assume the risks and release and hold harmless Choker Wrestling Club, Sports Coaching Associates and their agents, sponsors and volunteers who (through negligence or carelessness) might otherwise be liable to me, my minor child (or our heirs or assigns) for damages.
I attest that I am 18 years old or older and that my child is physically fit and have no known medical conditions which prohibit participation in this sport. My child and I agree to follow all laws, rules and guidelines regulating the conduct of the league, camps and clinics. I understand and agree that my child and I are responsible for the mechanical and/or operating condition of any and all sporting equipment provided by my child or by me for my child's use, and I agree that my child and I will continuously inspect and maintain all equipment used even if we have obtained any of the equipment from Choker Wrestling Club, their agents, sponsors and/or volunteers!
I have CAREFULLY READ this agreement and fully understand its contents. I am aware that this is a release of liability for myself and my child and a contract between myself, my child and Choker Wrestling Club and their agents, sponsors, and volunteers, and I have signed it of my own free will.
I also agree that my child will follow the wrestling program that is being taught during class that is not scheduled as open mat. All parents will encourage good sportsmanship by demonstrating positive support of all wrestlers, coaches, and officials at every practice and tournament.
I/We have read, understand and agree to comply with the Liability Waiver as outlined above.
