Instructions for Completion of
Container Manufacturer Certification Form


General Product/Container Data
Column 1: Provide a general description of each generic product category the company sells. Examples of a generic product are, motor oil, brake fluid, sealant, paint, cleaner, etc. It is not necessary to list all brand names within a category. Please use a separate line for each generic product category. Note: this information should be supplied by the product manufacturer.
Column 2: Indicate if the container is a bottle, jar, pail, tub, drum, etc. and the size of the container in fluid ounces or its equivalent volume. Please use a separate line for each container size used for a product category. For example, a 16 ounce bottle on line 1 and a 15 liter pail on line 2.
Column 3: Indicate the number of containers sold to the product manufacturer for each product or container category.
Column 4: Indicate the resin type for each product/container category. You can use either resin name or number. For example, high density polyethylene can be shown as "HDPE" or "2."

Columns 5 and 6: Post Consumer Resin Compliance Option
The container manufacturer must complete this section. The data should be supplied on a per container weight basis.
Column 5: Indicate the weight of plastic resin of each container for each product and/or container category.
Column 6: Indicate the weight of post consumer resin of each container for each product and/or container category.

Columns 7 and 8: Source Reduction Compliance Option
The container manufacturer must complete this section. The data should be supplied on a per container weight basis.
Column 7: Indicate the weight of plastic resin before the container was source reduced.
Column 8: Indicate the weight of the container after source reduction.
