ASPECT OF CARE – REVIEW CRITERIA
ALTERED LEVEL OF CONSCIOUSNESS
100% of all trip sheets of patients with Altered Level of Consciousness shall be reviewed to include the following Aspects of Care.
Documentation of:


Physical exam findings to include: neurological functions (ie – level of consciousness, pupillary response, modalities, sensation /perception, ability to follow verbal commands.) This aspect of care would exclude CARDIAC ARREST.
BLS REVIEW CRITERIA:


  • Vital signs

  • Pupillary response

  • Response to verbal or tactical (painful) stimuli

  • Airway management (to include oral or nasal airway as tolerated, oxygen application, assist ventilations if respirations are inadequate)

  • Secretion control

  • Positioning of patient to prevent aspiration

  • Administration of oral glucose if patient airway is maintained and patient is suspected to hypoglycemic.


ALS REVIEW CRITERIA (includes all of BLS criteria):


  • Airway management with endo or nasal tracheal intubation as indicated / oxygenation

  • Pulse oximetry (as part of vital signs)

  • Shock management with Patent IV lifeline and fluid replacement

  • Administration of Nalaxone or Dextrose as indicated

  • Fingerstick glucose level
